The short answer
Epidiolex is a prescription medicine made of purified CBD, approved by the FDA to treat seizures in three rare forms of epilepsy. Over-the-counter CBD products are not approved to treat any condition, aren't held to drug-level standards, and are typically used in much smaller amounts. They are not substitutes for each other. This article explains research, not a treatment recommendation; retail CBD is not a substitute for medical care.
What is Epidiolex?
Epidiolex is an oral solution of highly purified CBD. It's the only CBD product the FDA has approved as a drug. (FDA)
The FDA first approved it in June 2018 for seizures linked to Lennox-Gastaut syndrome and Dravet syndrome, two rare and severe forms of childhood-onset epilepsy. In 2020 it was also approved for seizures linked to tuberous sclerosis complex. It's approved for patients aged 1 and older. (Drugs.com approval history)
Its approval was based on randomized, placebo-controlled trials. That's the strongest kind of evidence that exists for any CBD product.
How is it different from over-the-counter CBD?
| Epidiolex | Over-the-counter CBD | |
|---|---|---|
| How you get it | Prescription only | Sold without a prescription |
| FDA status | Approved drug for specific seizure disorders | Not approved to treat any condition |
| What it's for | Treating seizures in three epilepsy syndromes | General wellness routines |
| Typical amount | Prescription dosing by body weight: 10 mg/kg/day maintenance for Lennox–Gastaut or Dravet syndrome, up to 20 mg/kg/day based on response and tolerability; 25 mg/kg/day for tuberous sclerosis complex. Divided twice daily. | Usually far smaller servings |
| Quality oversight | Drug manufacturing standards, uniform strength | Varies by brand; look for independent lab tests |
| Monitoring | Doctor supervision, including blood tests for liver enzymes | None required |
Why does the dose difference matter?
Epidiolex is dosed by body weight. For Lennox–Gastaut or Dravet syndrome, the recommended maintenance dose is 10 mg/kg/day, with increases up to 20 mg/kg/day based on response and tolerability. For tuberous sclerosis complex, the recommended maintenance dose is 25 mg/kg/day. Doses are divided twice daily. For a 30 kg (66 lb) child, 20 mg/kg/day equals 600 mg a day. These are prescription-label facts, not dosing guidance for retail CBD.
At those doses, doctors watch for side effects such as sleepiness, reduced appetite, diarrhea and raised liver enzymes, and they order blood tests to check the liver. (Drugs.com)
Can over-the-counter CBD be used instead of Epidiolex?
No. Over-the-counter CBD has not been tested or approved for seizures, and its strength and purity vary between brands. The FDA notes that unapproved CBD products haven't been evaluated for whether they work, what the right dose is, or how they interact with other drugs. (FDA)
If you or a family member has epilepsy, only use CBD under the guidance of your neurologist, and never change seizure medications on your own.
So what is over-the-counter CBD for?
People use over-the-counter CBD as part of everyday wellness routines, for things like winding down in the evening or after exercise. If that's what you're looking for, choose products with a Certificate of Analysis (COA), an independent lab report showing exactly what's inside.
